3 Simple Steps to Stay Covered

Don't Lose Your Health Coverage

Renewing your Medi-Cal is essential to keeping access to the healthcare services you and your family rely on, including doctor visits, prescription medications, preventive care, mental health services, specialist referrals, and emergency care.

ACE Overcomers' Community Coverage Connection Program provides free assistance to help Merced County residents successfully renew their Medi-Cal coverage and avoid unnecessary interruptions in healthcare benefits.

1. Update Your Contact Information

 

Make sure your county Medi-Cal office has your current address, phone number, and mailing information so you receive important renewal notices.

 

2. Watch for Your Renewal Notice

 

Check your mail regularly and carefully review any letters or renewal packets sent by your county Medi-Cal office.

3. Complete Your Renewal on Time

 

If additional forms or information are requested, complete and return them before the deadline. Missing your renewal deadline could result in a loss of coverage.

ACE Overcomers' Community Health Worker (CHW) Program helps individuals and families improve their health, access healthcare services, and connect with community resources. Our Community Health Workers are trusted members of the community who provide education, support, advocacy, and guidance to help you navigate health and social service systems with confidence.

How a Community Health Worker Can Help

Community Health Workers provide personalized support to help you:

  • Understand your healthcare benefits and services
  • Navigate medical appointments, referrals, and treatment plans
  • Connect to community resources for food, housing, transportation, and other basic needs
  • Manage chronic health conditions such as diabetes, asthma, high blood pressure, and heart disease
  •  Improve health literacy and make informed healthcare decisions
  • Access behavioral health and wellness resources
  • Receive support in English and Spanish

Community Health Champions Initiative

The Community Health Champions Initiative empowers Medi-Cal members and service providers to become trusted community leaders, health advocates, and wellness ambassadors. Participants receive training in health education, Medi-Cal resources, leadership development, and community engagement while helping improve the health and well-being of families throughout Merced County.

Who Can Participate?

  • Medi-Cal Youth & Young Adults (Ages 17–24)
  • Perfect for youth interested in leadership development, health careers, volunteer experience, and strengthening college or scholarship applications.
  • Medi-Cal Adults, Parents & Guardians (Ages 25+)
  • Ideal for adults who want to support healthier families, share lived experience, and make a positive impact in their community.
  • Medi-Cal Family Service Providers
  • Including medical providers, Community Health Workers (CHWs), Enhanced Care Management (ECM) staff, behavioral health professionals, Family Resource Specialists, and Community-Based Organization (CBO) staff.
